We are seeking a motivated, forward-thinking Critical Care Rehabilitation Coordinator to join our multidisciplinary Critical Care team based across both Doncaster and Bassetlaw hospital sites. This is an exciting opportunity to lead, develop and deliver high-quality rehabilitation pathways for patients recovering from critical illness.
Main duties of the job
You will play a key role in ensuring that rehabilitation is embedded across the patient journey–from admission through to post-ICU follow-up. Working closely with physiotherapists, occupational therapists, nursing teams, psychologists, and medical staff, you will champion personalised, holistic recovery and help improve outcomes for some of our most vulnerable patients.

- Significant post registration clinical experience within critical care, high dependency units or complex rehabilitation
- Experience working within MDTs, coordinating care across disciplines and pathways
Desirable
- Experience leading service development, quality improvement, or pathway redesign.
- Experience in family/carer support, patient follow up clinics, or post ICU recovery services.
Qualifications & Training
Essential
- Registered Healthcare Professional (e.g., RN, AHP such as Physiotherapist, Occupational Therapist, or equivalent).
- Current professional registration with NMC/HCPC
- Evidence of ongoing professional development relevant to critical care and rehabilitation
- Teaching/mentorship qualification
Desirable
- Post graduate qualification in Critical Care, Rehabilitation, Long Term Conditions, or Leadership.
- Accredited training in rehabilitation pathways, cognitive assessment, or functional assessment tools.
Personal Attributes & Skills

- Advanced assessment skills for physical, cognitive, and psychological rehabilitation needs
- Ability to design and coordinate individualised rehabilitation programmes
- Ability to work autonomously, make complex clinical decisions, and escalate concerns appropriately.
