Essential Car Documentation – This post is a designated Essential Car User. Therefore, you must always hold a full valid driving licence and provide your own car for use at work. You will receive the Essential Car User Allowance.

Medical Clearance – Pre-placement health assessment (PPHA)

Qualification – Hold or willing to undertake and achieve NVQ Level 4 in Health and Social Care within six months of commencing in post.

DBS (Disclosure and Barring Service) clearance – this post is covered by the Rehabilitation of Offenders Act 1974 (Exceptions) Order 1975 (amended in 2013). Appointable candidates will be required to apply for an Enhanced Disclosure and Barring Service (DBS) check. Information provided by you or the DBS will be dealt with in a confidential manner and in accordance with the DBS Code of Practice which can be viewed . We have a policy on the Employment of Ex-Offenders and it is available upon request.

This is an exciting opportunity to join Manchester’s Reablement service as a Reablement Team leader D2A Assessor. As a D2A assessor you will work within the Manchester Local care Organisation (MLCO) as a (Company Name - Subscribers only) Employee.

The Reablement Team offer an efficient and consistent approach to support which maximises potential for individuals to achieve reductions in care but also ensures consistency and high standards of service delivery.

As a Reablement Team leader you will organise and manage a team of reablement staff that provide direct care to customers in their homes in order to help them regain, or maintain, their independence.

You will ensure that the capacity of the team is managed effectively, through rota and performance management, in order to ensure demand for reablement services can be met.

Working collaboratively with Directorate colleagues, and Health Service colleagues, you will develop and deliver effective reablement and prevention services and implement a city wide approach in relation to the provision of community-based services in line with the Care Quality Commission (CQC) standards.

Communication Skills to demonstrate an understanding of the views of others and communicates in a realistic and practical manner using appropriate language and medium, listens attentively to views and issues of others and responds to issues arising.

Ability to plan and organise own time effectively, creating own work schedules, prioritising, preparing in advance and setting realistic timescales for own self and others.

Ability to use ICT systems to obtain and analyse data and present it effectively through a variety of ICT channels.

Problem Solving and Decision Making and an ability to analyse situations, diagnose problems, identify the key issues, establish and evaluate alternative courses of action and produce a logical, practical and acceptable.

Ability to lead, manage and motivate staff to high levels of performance in order to achieve change and maximise staff potential and contribution to the achievement of identified aims and objectives. Can also lead and plan the work of the team which deals with more diverse issues.

Ability to work confidently with financial data and make decisions using appropriate financial information, interpreting trends and accounting for risk.

Hold or willing to undertake and achieve NVQ Level 4 in Health and Social Care within six months of commencing in post.

Experience of working in a Social Care or Health Care Setting with an understanding of relevant Social Care legislation.

Full driving license and access to a car for business use.
