An exciting opportunity has arisen for Placement Officers within the Integrated Control Room within the Manchester Local Care Organisation.
You will play a key role in the identification of suitable care packages and placements. The role ensures the receipt of accurate and detailed information from community and hospital teams and uses this information to ensure the matching of resources available, to that of the need of the citizen. About the Candidate
We are looking for someone that is enthusiastic, resilient, and conscientious and is a team player. Someone with great organisational skills, used to working in a fast-paced environment and able to manage and prioritise changing workloads.
You will have a good working knowledge of IT systems and be confident in the daily use of a range of systems to ensure accurate data recording is maintained.
You will be a confident communicator both verbal and written.
The role holder will deliver a centralised placement function service and will play a key role in the identification of suitable placements/care packages, acting as a Single Point of Contact. The role holder will ensure the receipt of accurate and detailed information from teams and use this information to ensure the matching of resources available to meet the needs of citizens.
The role holder will work effectively as a member of the Brokerage Team; delivering specific areas of the Service and taking responsibility for the end-to-end processing and ordering all elements of individual support plans.
The role holder will need to liaise with and manage working relationships with commissioned providers and other stakeholders. The role will also involve contributing to contract monitoring and reporting as well as the implementation of Brokerage strategies.
