Following significant investment to improve patient flow across Leeds, we are recruiting enthusiastic Occupational Therapists to join our integrated Active Recovery and Neighbourhood Therapy Teams, delivering person-centred rehabilitation to adults at home.

You will support people with long-term conditions, provide falls prevention, deliver urgent responses to avoid hospital admission, and facilitate discharge using Home First principles to maximise independence.

Working holistically, you will use health coaching and self-management approaches, triage referrals, and ensure patients see the right professional at the right time while collaborating, delegating and signposting within the team.

You will develop clinical and leadership skills within a supportive multidisciplinary environment, with access to supervision, peer support and CPD. The role includes supporting assistants and students and contributing to service improvement. Managing your own caseload, you will work autonomously to coordinate rehabilitation and help patients achieve meaningful goals.

The service operates 7 days, 8am6pm, with enhanced pay for weekends. Flexible working is supported. Alignment to a North, South or West hub may include cross-cover. Visits welcome. The service functions over 7 days from 8am and 6pm. We promote a positive work/life balance and utilise flexible working to accommodate needs and have a post in the West of the city.

Main duties of the job

You will be responsible for your own caseload, delivering holistic rehabilitation made meaningful by taking place in local communities, delegating to others to optimise outcomes.

You will have opportunity to make a positive difference to your patients lives and develop skills in triage, rapid response, management of deteriorating patients and long-term conditions. You will work closely with the wider multidisciplinary team to enhance your knowledge, supported by clinical supervision, peer support and regular CPD.

You will develop leadership skills supporting others including therapy assistants and students and contribute to service improvement projects across the Trust and wider partners.
