We are looking for an enthusiastic, forward-thinking Team Manager to lead our Community Ready Unit (CRU) a key area supporting effective patient flow across the hospital. The CRU provides short-term care for patients who are community ready and no longer meet the criteria to reside, ensuring they continue to receive safe, compassionate, and well-coordinated care while awaiting discharge or community support. As Team Manager, you will provide strong, visible leadership to the multidisciplinary team, fostering a positive culture that prioritises patient experience, dignity, and timely discharge. You will be responsible for the day-to-day management of the unit, ensuring safe staffing, effective communication, and efficient patient turnaround in line with Trust priorities. Collaboration is central to this role. You will work in close partnership with the Transfer of Care Hub (ToCH) Pharmacy, Site Matrons, Therapy Teams, and external partners such as Age UK, and wider Community Services to deliver seamless care transitions and promote independence. Your leadership will help ensure that patients move safely through the hospital system supporting our ambition to be a modern, innovative Trust focused on continuous improvement and excellent patient outcomes. Main duties of the job
Provide day-to-day operational leadership for the Community Ready Unit, ensuring safe, effective, and compassionate care for patients who no longer meet the criteria to reside. Support and drive patient flow by coordinating timely admissions and discharges, working closely with Site Matrons, the Patient Flow Team, and community partners. Maintain oversight of staffing levels, skill mix, and allocation of workload, ensuring a supportive and efficient working environment for all team members. Lead effective multidisciplinary collaboration, developing strong working relationships with Pharmacy, Therapy Teams, Community Services, Social Care, and external organisations such as Age UK. Take a proactive role in Clinical Governance, promoting a culture of continuous learning, safety, and quality improvement. Participate in audits, incident reviews, and lessons learned processes to enhance standards of care. Monitor and improve performance through the use of data, audits, and quality improvement initiatives contributing to the Trusts modern, forward-thinking approach to care delivery. Support professional development and wellbeing of staff, providing regular supervision, appraisals, and opportunities for learning. Represent the Community Ready Unit at relevant Trust and system-wide meetings, promoting collaborative, patient-centred working across services.
