An exciting opportunity has arisen to employ a full-time Community Support Worker within the Neighbourhood Mental Health Team. This role will focus on the delivery of population-based mental health care for the community of Droitwich, based in our new suite within Droitwich Medical Centre. You will be working alongside a team of highly specialist practitioners and have direct relationships with the professionals within the local Primary Care Networks (PCN). If you are seeking a role that encompasses new ways of working that will be the future delivery of community mental health care in England, then this is an opportunity you should not miss. Consideration will be made for flexible working and there may be some opportunities for staff members to work from home where clinically possible.

Main duties of the job

Will work as a key member and resource of the MDT core team.

Aspects of the role would also include close working within the MDT, undertaking and supporting individuals accessing the service in Recovery focussed activities.

There is scope for professional development as the trust has a repertoire of training available to those wishing to extend their skill set. Developing our workforce is important to us, and we are pioneering champion roles across the organisation.

Support registered professionals to deliver evidenced based interventions, managing a caseload of patients

Supporting and signposting to improve social isolation and improved population health.

You will have experience of working with this user group in community settings.

The ability to work flexibly as the need of the service demands is essential.

Attend weekly MDT meetings, participate in professionals meetings, signpost to adjunct services depending on patient need.

Support and guidance to the carers of patients accessing the services.
