Overview The Care Coordinator plays a vital role in ensuring that service users receive comprehensive and coordinated care. This position involves working closely with healthcare providers, service users, and their families to facilitate effective communication and streamline care processes. The ideal candidate will possess strong organisational skills and a keen attention to detail, as well as a passion for improving service users’ outcomes.
Duties
- Act as the primary point of contact for service users, families, management, and healthcare providers to ensure seamless communication throughout the care process.
- Coordinate the rota, referrals, and follow-up care for service users, ensuring all necessary information is relayed effectively.
- Maintain accurate and up-to-date service user records in compliance with relevant regulations and policies.
- Assist in the development of care plans tailored to individual service users’ needs, collaborating with multidisciplinary teams.
- Monitor service users’ progress and provide support to address any barriers to care.
- Conduct regular check-ins with service users to assess their satisfaction with services and identify any additional needs.
- Coordinate all training for care staff.
- Assist management in conducting supervisions, spot checks, and all surveys.
- Assist the registered manager in completing audits.
- Ensure ECM is completed and up-to-date to facilitate the completion of monthly invoices.
Experience
- Previous experience in a domiciliary care agency and office setting is essential; familiarity with domiciliary care environments is advantageous.
- Strong organisational skills with the ability to manage multiple tasks simultaneously while maintaining attention to detail.
- Excellent communication skills, both verbal and written, enabling effective interactions with diverse populations.
- Proficiency in using office software applications and electronic ECM record systems is preferred.
- A compassionate approach towards service user care, demonstrating empathy and understanding in all interactions.

Pay: £22,629.00-£24,000.00 per year
