We are seeking a Full Time (37.5 hours) Co-Occurring Conditions Recovery Worker to join our West Hampshire No Wrong Door Team, working across Winchester and Andover on a fixed term basis of 18 months.
The No Wrong Door service is an innovative and exciting opportunity to provide a bridge between Primary Care, Community Mental Health services and Substance Use services.For too long, people who have co-occurring conditions have been referred around between services, none of which meets their overall needs for support.
No Wrong Door aims to change this by using a person-centred approach to ensure that appropriate agencies are involved in a person’s care when they need it the most.
This service has been developed as a result of an unmet need identified for those members of the public who struggle with the co-occurring conditions of poor mental health and substance use.
Main duties of the job
- Liaising with partner agencies in Primary Care, Community Mental Health care and substance use services
- Facilitating joint assessments with partner agencies and the individual
- Formulating joint care plans with partner agencies and the individual
- Ensuring that people receive the most appropriate care at the most appropriate time
- Advocating for clients with partner agencies
- Embracing the concept of recovery and demonstrating this in your clinical work
- Ensuring that your work is evidence based, up to date and in accordance with current policy and legislation.
- Keeping up to date and accurate records and documentation.

We recognise that a hybrid of both skillsets may not be available, and would support the successful candidates in gaining the knowledge required to undertake the role effectively.
